This session will include discussion on how practices can deploy elements of SDOH into patient messaging/marketing, member intake and how, in the context of onsite primary care centers, social determinants are leveraged to provide a full-spectrum of care both in and outside of the practice, includes elements of asset mapping and identifying/connecting patients with community health resources.
About Our Speaker:
Kristin Myren has held roles in the direct primary care and corporate health industry for nearly 10 years. She currently serves as an Associate Vice President for Premise Health, where she leads a team of 200 individuals with strategic and leadership oversight of 25 medical practices encompassing primary care, behavioral health, physical therapy, occupational health, acute care and more. Notable projects include leading the rollout of the COVID-19 vaccination program for her organization in 2020, holding lead roles in the opening of 11 medical practices and supporting numerous sites through mergers and acquisitions. Kristin holds an MBA in Health Administration from the University of Colorado, a Bachelor of Science in Public Health from the University of Arizona as well as PMP and Six Sigma credentials. Outside of work, Kristin enjoys volunteering for a number of health-oriented nonprofits, gardening, travel, and running. She has lived in Colorado for 7 years and is originally from Illinois.
